For a while Cynthia Webster for Cyndustries Modular Synthesizers company was making these but her module is no longer available. Currently similar (but inferior) pre-configured sequencers are available from Peter's Plan B company in the Module 21 series. Winning this auction you will get your Cynthia/Plan B sequencer you’ve been wanting; and you can configure the functionally well beyond anything offered commercially! Sequencer Configuration Choices
I was going for the fully-loaded setup. Of course, you can choose your own configuration and if less than mine you will have some spare parts.
My configuration was going to be:
6x 16 step CV banks
6x 16 step Programmable Pulse Bus
6x 16 step Stage Pulse Outs (output jacks not included)
1x 16 LEDs
Now that is one heck of an amazing analog sequencer! I was going to use the pulse outs to trigger drum sounds but you can use them for whatever you wish." via bug.out
Buyer beware - getting a version 1 of this working is going to take some tricks. There are quite a few errors in the schematic and on the pcb that require kludging, and getting the info from Peter is going to be difficult. There's no async in this version, which is one of the big attractions of this sequencer. Still, it's a good price for what you get, and it's become a rare item.

There is ASYNC directional VC. There is SYNC direction VC. What this doesn't have is ASYNC gate out of the obedient clock output
The schematics which shipped with this kit are accurate to it's rev level.
There are no errors on the schematic. The issue with the op amp inputs switched around was not a schematic error, it was my PCB error and they were corrected on the PCBs before they shipped !
The Milton Yahoo group has all the information anyone needs in as far the troubleshooting procedure - for instance the caps that MAY (not all cases, depends on the user's VCC levels) need to be added to the gate outputs.
There is a 25 page assembly instruction on line at the Big City Music site which has everything anyone will need to assemble one of these.
There is a bill of materials on the buzzclick site...it lists .01 bypass caps instead of .1, which don't makes no difference to the performance and is listed several times in the Milton Yahoo group.
There is a 25 step test procedure on the Buzzclick site. My email address is handily available. If you have a failure, write me the procedure steps which failed and I'll help you out.
