This one is from John L Rice.

"This is a great mixer in excellent shape! I built this one myself and modified it by adding toggle switches to mute the individual outputs. This is the issue/version #4 PCB, has a BrideChamber front panel and was built up as the most popular 'Stooge' version which uses three reversible attenuators for the inputs and three output jacks so this module can be used as a 3 to 1 mixer, a 2 to 1 mixer + a single attenuator or three single attenuators. Very versatile!

Note that the first three pictures are from 2009 when I had just finished building the module. It is still in almost as good of shape but some of the black is worn off around the mounting holes, which can't be seen when the module is mounted in a modular case, and the top knob has a slight scratch in it. I'll try to post a recent picture soon.

This module requires a clean +-15 VDC power supply and a case to mount it in, neither are supplied. A power cable and mounting screws are supplied though."
