MIDISID enhancements and a Mimijam using MIDIPan with MIDISID

"There are some MIDISID enhancements that I'd like to tell you about and demonstrate - improvements to pulse width modulation (PWM) and a visualiser for general MIDI mode. It's an excuse to have a jam using MIDIPan's looper and MIDI output plugged into MIDISID for those sweet sweet SID chip sounds.
