www.styleflip.com. They have a web based editor that is very good for designing your overlay but you can really customize your designs with a web based graphics editor like
www.pixlr.com. It helps to have objects in the design on separate transparent layers to adjust for knob placement.

They print your design on A 3m plastic which is about as thick as a playing card but pretty sturdy. The surface is textured and feels similar to the front of a DSI TETRA. I was worried that my design would end up looking like a lunch box but it still Looks and feels like a synth. I thought it was fun and satisfying to make one of these. I wish they had a template for the xoxbox."
